What families usually miss
- Movers deliver — they don't unpack, set up the bed, or arrange the room. That's family.
- Transportation from rehab/hospital on admission day is rarely included in facility transport.
- Wellness package fees are separate from the base monthly rent — and can change after assessment.
- Incontinence supplies are usually family-supplied unless you enroll in a specific program.
- Lost clothing and hearing aids are common — labeling is your first defense.
- Care level increases can quietly raise the monthly bill — ask how changes are communicated.

- Designate Responsible Party and provide POA / healthcare surrogate
- Schedule the nursing assessment that determines wellness package level
- Transfer or set up local pharmacy aligned with facility's med system
- Request copy of physician orders, current med list, and allergies
- Measure room — confirm bed size, furniture fit, TV mount options
- Ask: who unpacks and sets up the apartment on move-in day?This is the #1 surprise. Movers usually only deliver.
- Confirm transportation FROM hospital/rehab/SNF — facility transport rarely covers admission day
- Order labels for clothing, glasses, hearing aids, walker, chargers
- Update mailing address, pharmacy, doctors, family contacts
- Review prohibited items list (space heaters, certain meds, weapons, etc.)
- Label every clothing item — laundry mix-ups are the most common complaint
- Pack a First-Night Bag (briefs, wipes, toiletries, meds, chargers, pajamas)
- Print medication list, insurance cards, POA, advance directives — bring originals + copies
- Charge hearing aids, phone, tablet, emergency pendant if personal
- Bag bedding and towels separately — facility may not have linens ready day one
- Pre-arrange furniture delivery window — coordinate with facility move-in slot
- Prepare a small framed family photo and one comfort item for the bedside
- Fill 30-day supply of every medication
- Hand-deliver medications to nursing — get signed receipt
- Confirm emergency pendant works and resident knows how to use it
- Set up bed, clear walking paths, position favorite chair within reach of call button
- Place glasses, hearing aids, water, tissues, remote on bedside table
- Test thermostat, TV, Wi-Fi, refrigerator, microwave, phone
- Walk the route from room to dining room and to nearest bathroom together
- Meet the shift nurse and at least one caregiver by name — write them down
- Confirm first meal time and whether someone will escort your parent
- Take photos of the room setup and a list of items left behind
- Stay through dinner the first night if possible
- Visit at different times of day — morning, mealtime, evening
- Confirm medications are being given on schedule
- Verify shower/toileting assistance is happening as planned
- Check that laundry has been picked up and returned
- Ask your parent: do you know how to call for help?
- Note any items already missing — clothing, hearing aids, dentures
- Introduce yourself to the Director of Nursing or Wellness
- Set up automatic delivery for incontinence supplies if not facility-provided
- Attend the 30-day care plan meeting — bring written observations
- Review first invoice line by line — flag any unexpected charges
- Confirm care level hasn't been quietly increased without notice
- Replenish consumables — briefs, wipes, toiletries, snacks
- Replace any lost clothing and re-label new items
- Schedule outside appointments (dentist, podiatrist, audiologist) — confirm transport
- Document mood, sleep, appetite, and cognition changes
- Ask about social engagement — which activities did your parent attend?
- Build relationships with the regular caregivers — they are your eyes and ears
